Prof. Samsuridjal said the possibility of infection experienced by people living with HIV in Indonesia is tuberculosis (tb), fungal infections, toxoplasmosis, cryptococcosis, CMV eye infections and viral hepatitis co-infection.
"Opportunistic infections are usually associated with a CD4 count in the body. When the CD4 count is small, then an infection that may arise tend to be more severe," said a doctor who also joined in a special study group AIDS-FKUI Cipto Mangunkusumo.
CD4 value indicates the value of immunity / immune / endurance as indicated by T cells in the blood. Generally, the category of AIDS if CD4 values below 200 and with numbers under it will begin to opportunistic infections.
Opportunistic infections that may arise in a person with HIV / AIDS, namely:
1. Tuberculosis
If the value is more than 200 CD4 cells/mm3 the same signs and symptoms associated with tuberculosis, but if the value is less than 200 CD4 cells/mm3 and radiologinya the clinical picture is not typical. Treatment was with ARV therapy and TB are adjusted by the number of CD4nya.
2. Cryptococcal meningitis
This condition is usually characterized by headache, decreased consciousness, values less than 100 CD4 cells/mm3 and examination results show keriptokokus antigen positive. Therapy was given amphotericin B, fluconazole high dose.
3. Diarrhea
In people with HIV / AIDS often encountered experiencing chronic diarrhea due to pathogenic bacteria infection. In this condition rehydration therapy is essential and stool examination was also conducted against bacteria, fungi, parasites or viruses.
4. Cytomegalovirus
This condition is often found in people who have a value of less than 50 CD4 cells/mm3 and viral reactivation as a result of existing in the body. These infections can affect the retina, gastrointestinal tract, respiratory tract and brain.
5. Cancers associated with HIV
Cancer that might arise is the lymph node cancer, cervical cancer and Kaposi sarcoma.
6. CMV infection
This condition begins with reduced visual acuity, followed by like there was a flash of light, misty vision, visual field disturbances, and if progressives can lead to blindness within 2 months without treatment. Usually occurs if the value is less than 50 CD4 cells/mm3. Therefore should be detected as soon as possible prior to the retina or macula.
7. Chorioretinitis Toxoplasmosis
A person who experiences this condition will have symptoms of blurred vision, pain and seen a black shadow on the fly-eye flight. Needed to diagnose clinical findings and serological tests.
"Principles for the management of HIV / AIDS is by taking antiviral drugs (ARVs), treatment of opportunistic infections and basic treatment. While supportive treatment can be done is to improve the general state of PLWHA, pay attention to nutrition, psychosocial support and symptomatic drugs so that PLWHA can do activities," explains Prof. Samsuridjal.
